These Terms & Conditions ("Terms") govern your use of the website, contact and appointment forms, and SMS text messaging program ("Service") offered by Davin Li Dental Corp, d/b/a Transformation Dental Partners ("we," "us," or "the Practice"). By submitting a form on our website or opting in to receive text messages, you agree to these Terms.


1. SMS Messaging Program


As part of the Service, the Practice offers text message (SMS) communications to patients and prospective patients who opt in. There are two categories of messages you may receive, depending on which consent option(s) you select:


  • Appointment & Account Messages: appointment reminders, appointment confirmations, and related scheduling communications.
  • Marketing & Promotional Messages: special offers, discounts, new service announcements, and related promotional content.


Message frequency varies depending on your appointment activity and the option(s) you opt in to. You may opt in to one or both categories independently; opting in to one does not require opting in to the other.


2. Opt-Out Instructions


You may opt out of receiving SMS messages from the Practice at any time by replying STOP to any message you receive. After you send STOP, we will send one final message confirming that you have been unsubscribed, and you will receive no further messages from that program unless you opt back in. If you are enrolled in more than one messaging category, you may need to reply STOP separately to each, or contact us directly using the information in Section 3 to be removed from all programs.

Message and data rates may apply to any SMS messages sent or received as part of this Service, depending on your mobile carrier and plan. Message frequency varies. Carriers are not liable for delayed or undelivered messages. Standard messaging rates apply based on your mobile carrier's rate plan.


5. Carrier Liability Disclaimer


Wireless carriers (including but not limited to AT&T, T-Mobile, Verizon, and other participating carriers) are not liable for delayed or undelivered messages. Carriers do not guarantee delivery of any SMS messages sent through this Service and are not responsible for any content, errors, or omissions in messages sent by the Practice.


6. Age Restriction


You must be at least 18 years of age to opt in to this Service and to submit forms on this website on your own behalf. By opting in, you represent and confirm that you are 18 years of age or older, or that you are the parent or legal guardian of the patient on whose behalf you are submitting information, and that you are authorized to consent to these Terms and to the collection described in our Privacy Policy.
